About the Role

This position provides in-office case management services to families within the residential shelter program. Responsibilities include evaluating and assessing immediate needs, overseeing accurate data quality of client files, and connecting families with services and resources.

Responsibilities

  • Provides weekly case-management services to assigned clients in the Residential Shelter Program, including developing client support plans, goals, and resources, and maintaining confidential client case notes, history, and data through HMIS (Unity).
  • Oversees data quality of all assigned client files upon entry and exit from the program.
  • Coordinates Sanctuary groups in the Residential Shelter.
  • Monitors monthly coordinated entry follow-ups and verifies updates within the first week of each month.
  • Participates in staff and group meetings.
  • Organizes and facilitates meetings for child and family-related decision-making, including Family Team Conferences, Red Flags, and House Celebrations.
  • Maintains client case notes, history, and data with confidentiality best practices.
  • Generates and compiles client outcomes and reports within the electronic record platform for funder reporting.
  • Keeps confidential records according to the Code of Ethics.
  • Maintains minimum weekly contact to track family progress.
  • Maintains professional boundaries.
  • Monitors grant-required monthly outcomes reports for residential programs, including Coordinated Entry, exit destinations, and benefits verification.
  • Provides training to interns to ensure proper case management guidelines are followed.
  • Visits and meets with community service providers, organizations, and institutions beneficial to maintaining self-sufficient living.
  • Participates in Metropolitan Ministries’ holiday efforts and fund-raising events such as Bridge Builders when needed.
  • Supports the team in other tasks as assigned and attends all team meetings.
  • Meets personal goals and actively contributes to the achievement of team and organizational targets annually.

Requirements

  • Bachelor’s degree in Social Work or Human Services field preferred.
  • Knowledgeable about homeless issues, with experience working with individuals with mental health/substance abuse issues or willingness to seek continuing education related to the homeless population.
  • Bilingual (Spanish) preferred.
  • Ability to work within a multi-disciplinary team framework; strong interpersonal skills.
  • Strong oral and written communication skills.
  • Dependability, maturity, and sensitivity to cultural and individual differences in clients served.
  • Ability to handle job-related matters in a professional, diplomatic, and confidential manner.
  • Willingness to work with minimal supervision while accepting direction.
  • Well-organized and detail-oriented.
  • Heart for ministry and understanding of poverty.
  • High degree of maturity and strong interpersonal skills to work with low socio-economic and diverse populations facing crisis situations.
  • Ability to de-escalate clients on the edge of verbal or physical violence.
  • Computer literacy with knowledge of Microsoft Word and Excel.
  • Oral and written communication skills for report writing and conducting meetings.

Physical Requirements

  • Adequate hearing and speaking ability to participate in oral communication.
  • Capability to work outdoors in sunlight and higher temperatures occasionally.
  • Physical, emotional, and spiritual stamina to handle job-related issues and stress.
  • Ability to lift 15 lbs. or more.

Other Requirements

  • Legal authorization to work in the United States.
  • Must pass required pre-employment background screening.
  • Valid driver’s license, clean driving record, and ability to qualify for the Ministries’ driving program.
