Job Description & Requirements LCSW Social Worker - Hospice StartDate: 10/26/2026 Pay Rate: $1600.00 - $2500.00 POSITION SUMMARY – LCSW Social Worker - Hospice POSITION DUTIES – Provides psychosocial assessments, emotional support, counseling, care coordination, resource referrals, and end-of-life guidance to patients and their families to help them navigate the physical, emotional, financial, and social challenges associated with terminal illness.
