About the role
Responsible for providing comprehensive case management services to guests with the primary goals of maintaining housing, securing mainstream benefits, and increasing income and self-sufficiency. Case management activities include assessment of needs, development of long-term goals, provision of information and referrals, life skills training, and coordination of resources. All services must be fully documented, aligned with the mission of Presbyterian Night Shelter, and coordinated with relevant internal departments. Case management meetings will be conducted at scattered-site locations within residents’ homes and other community-based settings. The role also requires close collaboration with community partners, landlords, and internal departments to provide coordinated care that strengthens housing security and fosters client self-sufficiency.
Requirements
- Bachelor’s degree in Social Work or related field
- Experience working with diverse and/or difficult populations desired
- Experience working with the homeless population is preferred
- Experience working with individuals with mental illness is preferred
- Current knowledge of the local network of services preferred
- Knowledge of Microsoft Word and Excel
- Valid driver license and proof of auto liability insurance
Pay
$20.67 per hour
