Job Summary

Responsible for case management activities for residents residing at Crowley with the primary goals of maintaining housing and mainstream benefits along with increasing income and self-sufficiency. Case management includes providing information and referrals, life skills training, and long term goal planning with residents.

Responsibilities

• Provide in-unit case management sessions
• Assess resident needs
• Identify resources matched to the need
• Coordinate with residents to connect with services
• Document services provided
• Coordinate services with all relevant departments, ensuring they fall within the mission of Presbyterian Night Shelter

Qualifications

• Bachelor's degree in Social Work or related field is required
• Experience working with diverse and/or difficult populations desired
• Experience working with the homeless population is preferred
• Experience working with individuals with mental illness is preferred
• Current knowledge of the local network of services preferred
• Knowledge of Microsoft Word and Excel is required
• Valid driver license and proof of liability insurance is required

Pay

$21.15 per hour
