Job Coach (DSP) - Supported Employment - 30 hours/week

About the role

Days/hours can vary each month based on individuals' work schedules (can include weekends) – 30 hours/weekly.

Responsibilities

• Timely transport individuals to and from their place of employment.
• Assist in training, guiding and completing tasks as applicable at place of employment.
• Assist management team in identifying and providing opportunities for learning new skills and independence working with each individual’s goals and plans.
• Electronically record daily activities maintaining confidentiality and complying with all policies and procedures to protect the identity, medical, HIPAA information.
• Provide safe and nurturing environment where individuals are treated with dignity, respect, can exercise their rights and responsibilities and are protected from abuse or neglect.
• Obtain and maintain all required certifications and trainings (in person and online).
• Perform other duties as assigned.

Physical Efforts

• May include extended periods of standing and walking.
• Positions may require the ability to independently lift at least 25 pounds.

Requirements

• High school diploma or equivalent.
• At least age 18.
• Valid driver’s license, clean driving record back 3 years, and reliable transportation.
