We are seeking a compassionate and organized IRIS Consultant to support adults, older adults, and individuals with disabilities in Wisconsin's IRIS (Include, Respect, I Self-Direct) Program. Key Responsibilities Include Building trusted relationships with participants and assisting in developing personalized support plansConducting regular contact and in-person visits to monitor health, safety, and program outcomesPromoting participant choice, independence, and community inclusionEducating participants on program eligibility and available resourcesCollaborating with community organizations, healthcare providers, and service agenciesMaintaining accurate case documentation in compliance with standardsSupporting annual functional screens and working independently within a multidisciplinary team Qualifications Include Bachelor’s degree in Social Work, Human Services, Psychology, or related fields with at least one year of relevant experienceOr four or more years of direct service experience with older adults or individuals with disabilitiesOr current IRIS Consultant in good standing with Wisconsin DHS exemption Preferred Skills Experience in case management, care coordination, disability services, and long-term careStrong communication, relationship-building, and cultural awareness skillsProficiency with technology and ability to work independently with flexible schedulingBilingual candidates in languages such as Spanish, Hmong, Arabic, Russian, or ASL are encouraged to apply Benefits Offered Include Medical, Dental, and Vision insurancePaid time off, holidays, mileage reimbursement, retirement plan, and wellness programsAdditional support resources and eligibility for Public Service Loan Forgiveness Requirements Valid driver’s license, reliable transportation, and ability to travel within Milwaukee and surrounding areasSatisfactory driving record and insurance compliance If passionate about social services, case management, and empowering individuals, this role offers an opportunity to make a meaningful impact through person-centered support and community engagement.
