We are seeking a compassionate and organized IRIS Consultant to support adults, seniors, and individuals with disabilities in Wisconsin's IRIS program. Key Responsibilities Include Building trusting relationships with participantsDeveloping and updating individualized support plansConducting regular contact and in-person visitsPromoting self-direction, independence, and community involvementMonitoring health, safety, and program outcomesEducating participants about eligibility and resourcesCollaborating with community agencies and providersMaintaining accurate case documentationSupporting annual functional screens and working independently within a team Qualifications Bachelor's degree in relevant fields with at least one year of experience working with older adults or individuals with disabilities, or four+ years of direct service experience, or current IRIS Consultant status Preferred Skills Case management, care coordination, disability or long-term care experienceStrong communication, cultural awareness, and ability to work independentlyBilingual abilities in languages such as Spanish, Hmong, Arabic, Russian, or ASL are highly valued Additional Requirements Valid driver's license, reliable transportation, and ability to travelProficiency with technology and commitment to participant rights This role offers comprehensive benefits and the opportunity to empower individuals to live self-directed, independent lives through community-based support.
