IRIS Consultant (53072)

We are seeking a compassionate and organized IRIS Consultant to support adults, older adults, and individuals with disabilities within Wisconsin's IRIS program. This role involves case management, care coordination, social work, and community-based support to help participants maintain independence, achieve personal goals, and exercise self-direction. Responsibilities include developing and updating individual service plans, conducting regular contacts and visits, promoting participant choice and community inclusion, monitoring health and safety, educating clients on program details, and collaborating with healthcare providers and community organizations. The position requires strong relationship-building, communication skills, and the ability to work independently while maintaining accurate documentation within compliance standards. Preferred qualifications include experience in case management, disability services, or long-term care, along with cultural awareness and bilingual abilities in languages like Spanish, Hmong, or ASL. Additional requirements include a valid driver's license, reliable transportation, and the ability to travel within Milwaukee and surrounding areas.
