IRIS Consultant (53007)

We are seeking a compassionate IRIS Consultant to support adults, older adults, and individuals with disabilities in Wisconsin's IRIS (Include, Respect, I Self-Direct) Program. Key Responsibilities Include Building trusted relationships with participants and developing individualized support plansConducting regular contact, in-person visits, and monitoring health, safety, and program outcomesPromoting self-determination, independence, and community inclusionEducating participants about program requirements and available resourcesCollaborating with community organizations, healthcare providers, and service teamsMaintaining accurate documentation and ensuring compliance with standardsAssisting with functional screens and supporting participant goals Qualifications Include Bachelor's degree in Social Work or related field with at least one year of experience working with related populations, or four+ years of relevant experienceStrong communication, relationship-building, and cultural awareness skillsAbility to work independently with a flexible schedule and proficiency in technologyValid driver's license, reliable transportation, and willingness to travel within the region Preferred skills: case management, care coordination, disability and aging services, Medicaid, behavioral health, and person-centered planning.
