Intermediary Care Program Coordinator

Essential Functions

  • Oversee assigned public health program area;
  • Supervise staff in program area;
  • Mentor program staff and provide guidance and direction to complete assignments;
  • Prepare written reports and oral presentations;
  • Stay informed on best practices in program area;
  • Develop educational materials related to assigned program area (fact sheets, brochures, newsletters, etc.);
  • Prepare grants, develop funding proposals and budgets;
  • Perform job duties in a way that promotes a positive image of the City organization and delivers exemplary customer service skills;
  • Keep managers and department director apprised of activities in assigned program area;
  • Perform work during emergency, disaster or communicable disease incidents 24/7 as required;
  • Perform other duties as assigned.

Qualifications

  • Bachelor's degree in a social science or related field with an additional 2 years’ experience in public health.
  • Master's degree in Public Health or related field preferred.

Physical Requirements

  • Frequently lift up to 10 pounds and occasionally lift up to 25 pounds;
  • Frequently carry up to 10 pounds and occasionally carry up to 25 pounds;
  • Occasionally stand and walk;
  • Occasionally kneel;
  • Occasionally pull or push objects;
  • Occasionally rotate upper trunk to right or left while sitting or standing;
  • Occasionally reach above, at, or below shoulder height;
  • Frequently handle objects;
  • Cordinate eye, hand, and foot movement in order to operate a vehicle.
