About the role

The Interim Continuum of Care Executive will provide executive leadership across a comprehensive continuum of post-acute services, ensuring operational alignment, quality care, and consistent performance across multiple programs.

  • Assess current leadership structures, establish clear expectations and accountability, and provide mentorship to strengthen operational effectiveness across the continuum.
  • Develop and execute strategic priorities that support organizational growth, quality, patient experience, and sustainable delivery of care across post-acute services.
  • Partner with executive and clinical leadership to identify opportunities for process improvement, strengthen care coordination, and ensure effective implementation of strategic initiatives.

Ideal candidates are experienced post-acute care leaders with a strong background in hospice services, strategic planning, organizational transformation, and leading diverse teams through periods of change. Must be available to start within 2–3 weeks of acceptance.

Requirements

  • Bachelor’s degree is required.

Pay

Compensation Range: $225,000 to $240,000 annually. The final compensation rate will be determined based on experience, education, training, location, internal equity, and budget considerations.

Interim positions include coverage for all travel, accommodation, and work-related expenses. Bi-weekly trips home at the client's expense, plus a rental car and comfortable lodging, are provided.
