Key Responsibilities
- Provide comprehensive case management, care coordination, and advocacy for clients receiving recovery services
- Develop, implement, and update individualized service plans to address client needs and monitor progress
- Career care services within the continuum of care
- Assess client needs and coordinate holistic care plans
- Maintain ongoing communication with clients, families, and support systems
- Connect clients with housing, legal, transportation, and community resources
- Document service coordination activities and maintain accurate records
- Build collaborative relationships with healthcare providers, community agencies, and civic organizations
- Participate in required meetings, trainings, outreach events, and continuing education
- Maintain required certifications and complete annual training requirements
- Ensure compliance with confidentiality standards and regulatory requirements
- Perform other related duties as assigned
Qualifications
- Bachelor's degree in Human Services or a related field
- Strong interpersonal, communication, and organizational skills
- Basic computer and data entry proficiency
- Fundamental understanding of healthcare services and medical terminology
- Prior experience in a medical or physician office setting
- Knowledge of Joint Commission standards
- Bilingual in Spanish, Nepali, or French (preferred)
Work Environment
- Office and community-based position supporting clients throughout the service area
- Frequent interaction with healthcare providers, community organizations, and client support systems
- May involve transportation assistance and community outreach activities
- Exposure to routine medical office and clinic environments while supporting individuals with behavioral health and recovery needs
