Intensive Case Manager

About the role

Responsible for providing intensive case management services to clients in need. Conduct assessments, develop plans, and coordinate resources to support client needs.

Responsibilities

• Conduct comprehensive assessments to identify client needs
• Develop and implement individualized service plans
• Coordinate with community partners to secure necessary resources
• Maintain regular contact with clients to monitor progress
• Document all interactions and outcomes in client files

Requirements

• Bachelor’s degree in Social Work, Psychology, or related field
• Minimum 2 years of relevant experience in social services or related field
• Valid driver’s license and access to reliable transportation
• Strong interpersonal and communication skills
• Ability to work independently and manage multiple tasks simultaneously

Qualifications

• Knowledge of mental health and substance abuse issues
• Experience working with diverse populations
• Proficiency in Microsoft Office Suite

• Excellent organizational and time management skills
• Strong problem-solving abilities
• Ability to work under pressure and meet deadlines
• Effective written and verbal communication skills

• $30-35 per hour

Schedule

• Full-time position
• Monday through Friday, 8:00 AM – 4:30 PM
