Intensive Case Manager for Adults
Requirements
- Bachelor’s degree in sociology, social work, psychology, or a related field; OR
- High school diploma/GED with 12 college credit hours completed in relevant coursework, plus two years’ experience in human services with one year in direct client contact; OR
- A registered nurse (RN) license.
- High school diploma/GED with 12 college credit hours completed in relevant coursework, plus two years’ experience in human services with one year in direct client contact; OR
- Valid driver's license and reliable transportation required.
- Experience working with adults in public or private human services is preferred.
Responsibilities
- Travel throughout Cumberland and Perry Counties to meet clients in their homes and communities.
- Deliver Intensive Case Management (ICM) services to adults with serious mental health needs.
- Conduct assessments, develop service plans, and provide ongoing support through linking, monitoring, advocacy, and skill-building.
- Support clients in accessing psychiatric care, community resources, and social supports.
- Provide education and support to families and significant others.
- Maintain accurate and timely documentation in compliance with Merakey Behavioral Health, County, and State regulations.
- Participate in clinical supervision, staff meetings, and quality improvement initiatives.
- On-call for caseload.
Qualifications
The ideal candidate will possess the following qualifications:
- Bachelor’s degree in sociology, social work, psychology, or a related field; OR

Pay
$20.50/hour or $22.50/hour with the selection of Enhanced Pay.
Schedule
Full-time, 40 hours per week, Monday–Friday. Typical schedules include 8:00 AM–4:00 PM or 9:00 AM–5:00 PM.
