Adult Intensive Case Manager

About the role

Under the supervision of the Clinical Program Manager, the Case Manager provides recovery-oriented case management services, including skill-building and medication education. This position conducts routine psychosocial and emotional needs assessments and develops Person-Centered Recovery Plans. The Case Manager works with adults and families to connect them with community resources, provide rehabilitative services, and initiate crisis intervention when necessary. All services are delivered in collaboration with a multidisciplinary recovery team and are documented in the electronic health record. These services are designed to promote recovery and wellness, support community integration, and enhance independent living and overall quality of life. The role requires detailed and accurate electronic documentation, as well as strict adherence to agency and state guidelines. At least 55% of working hours must be dedicated to direct service.

Responsibilities

• Prioritize needs during crisis situations
• Participate in regular meetings and training opportunities
• Perform other assigned duties with a positive and professional attitude

Qualifications

Minimum Qualifications:

• Bachelor's degree in Human Services
• If degree is not in Human Services, completion of a minimum of 24 credit hours in a human services-related field
• Strong understanding of mental health diagnoses and treatment
• Ability to work both independently and collaboratively in a fast-paced, recovery-oriented environment
• Proven ability to build meaningful connections with individuals served
• Valid Texas driver's license with ability to maintain approved driver status throughout employment

Preferred Qualifications:

• Master's degree in Human Services or a related field
• At least one year of experience providing wellness and recovery support services in mental health, substance use treatment, and/or crisis intervention and screening
• Actively pursuing clinical licensure in counseling, behavior analysis, social work, family therapy, or a related discipline (advanced or full licensure preferred)
• Bilingual proficiency in Spanish
• Experience with concurrent (real-time) documentation practices
• Current licensure as a Licensed Chemical Dependency Counselor (LCDC)

Pay

Hourly salary range: $21.46 - $28.71

• $1.00 per hour differential for 2 years previous experience
• Up to $4.00 per hour differential for BTCS tenure
• $0.75 per hour differential for bilingual
• $1.00 per hour differential for advanced licensure
• $0.50 per hour differential for LCDC
