About the role
The Gándara Mental Health Center offers career pathways in various areas including Behavioral Health, Substance Use and Recovery, Community Based Clinical services, Adolescent and Family services, Adult Residential, Maintenance, and Administration. Opportunities are available at both entry-level and leadership levels.
Responsibilities
- Facilitate care planning and coordination of services for youth with SED under the age of 21.
- Ensure necessary services are accessed, coordinated, and delivered in a strength-based, individualized, family/youth-driven, and culturally relevant manner.
- Develop individualized and flexible plans of care through a Wraparound planning process.
- Work with a Family Partner to develop an Individual Care Plan and a risk management/safety plan.
- Maintain a caseload of 11-14 families and oversee the assembly of the Care Planning Team.
- Provide or arrange for transportation for clients to appointments, crisis/respite services, etc., if needed.
- Provide or secure support and crisis/emergency services for the youth/family.
- Collaborate with other necessary individuals to properly coordinate care.
- Identify community resources and develop natural supports for youth and parents/caregivers to support and sustain achievement of the youth’s individual care plan, goals, and objectives.
Requirements
- Minimum of 1 year of experience.
- Bilingual and bicultural candidates encouraged to apply (Portuguese & English, Spanish & English).
- Must possess an MCE approved Master’s or Bachelor's Degree in Family Therapy, Counseling and Psychological Services, Human Services, Community Psychology, etc.
- Driver’s License and proper auto insurance required.
- Awareness and sensitivity to contextual variables such as race, culture, gender, sexuality, disability, economics, and lived experience.
Qualifications
- Master’s Level: Salary Range $64,000 - $68,000
- Bachelor Level: Salary Range $56,000 - $60,000
Skills
Knowledge of federal, state, and agency requirements for progress notes, comprehensive home-based assessments, and behavioral health assessments. Familiarity with the Massachusetts Child and Adolescent Needs and Strengths (CANS) assessment tool. Strong communication and collaboration skills. Ability to maintain confidentiality and handle sensitive information.
