Summary

ICC’s will create and oversee a process of enhancing supports, building on strengths and meeting needs for referred children and their families, using the wraparound planning process, trauma informed care, and Children’s System of Care principles; ICC will partner with CWS, Probation, group home providers, and other local community agencies; Average caseload is 10–12 youth.

Essential Duties and Responsibilities

  • Provides assessment, evaluation, plan development, collateral, crisis intervention, and intensive case management services
  • Guides the Wrap Team development and process, ensuring smooth transition through the four phases of the wraparound process and emphasizing inclusion of natural supports
  • Develops, coordinates, and ensures the implementation of the client plan
  • Maintains accurate, complete, and timely case records per Medi-Cal requirements and SDCC policies
  • Collaborates with CWS Social Workers, and Probation Officers, and outside resources including schools, psychiatrists, community outreach programs, family advocacy programs, and others as appropriate
  • Participates in fidelity and quality improvement efforts, including wrap coaching, ongoing training, and supervisions
  • Sustains the team commitment to Wraparound values and approach
  • Coordinates efforts with Parent and Youth Partners and other appropriate staff in providing supports and services
  • Maintains current licensure and or intern registration
  • Other duties may be assigned

Language Skills

  • Ability to read, analyze, and interpret medical records, journals, reports, and legal documents
  • Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community
  • Ability to effectively present information to management and public groups
  • Complete timely, organized documentation for trainings

Reasoning Ability

  • Ability to define problems, collect data, establish facts, and draw valid conclusions
  • Ability to interpret and extensive variety of instructions and deal with several abstracts and concrete variables

Physical Demands

  • The employee is regularly required to stand; use hands to finger, handle, or feel; reach with hands and arms; stoop, kneel, crouch, or crawl; and talk and hear.
  • The employee frequently is required to run, walk, sit and drive a vehicle.
  • The employee must be able to move and/or lift a child when clinically necessary and must be able to follow a child that AWOLs in a walking or running pace.
  • Applying SPA restraint techniques can involve lifting the child, holding the child down in a standing, sitting or ground level manner, as well separating clients who are acting in an unsafe manner to ensure the safety for the clients and staff.
  • Due to the emotional instability of the clients the employee may be subjected to kicking, hitting, punching, biting, hair pulling, etc.
  • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

  • The noise level in the work environment is usually moderate.

Qualifications

  • Bilingual in English and Spanish
  • Valid California Driver’s License
  • Master’s Degree from an Accredited University
  • Registered Associate or in process with the BBS in Social Work, Psychology, Professional Clinical Counseling, or Marriage and Family Therapy and License Eligible in the State of California
  • Must be highly organized, with excellent written and verbal communication skills
  • Experience working with high risk youth
  • Ability to work cohesively on a team
