About the role
JRI is seeking an Intensive Care Coordinator (ICC) to support youth under the age of 21 with serious emotional disturbances. The ideal candidate will have a talent for connecting with youth and their families and a desire to make a positive impact.
Responsibilities
- Facilitate Care Planning Teams
- Coordinate multiple services/supports for MassHealth-enrolled children and youth
- Act as a liaison with individuals, agencies, and programs
- Identify, assist families in accessing, and monitor available services and natural supports
- Provide trauma-informed, wrap-around services that center the needs of the youth
- Support parents and caregivers in learning effective advocacy skills
Requirements
- Bachelor’s or Master’s degree in social work, psychology, or related human service field
- Daily access to reliable, insured vehicle and active Driver’s License
- Flexible work schedule, including evenings and weekends
- Experience navigating child/family-serving systems and working collaboratively with state agencies, consumer advocacy groups, and behavioral health facilities
- Experience in working with and advocating for children/families with serious emotional/behavioral health needs
- Knowledge of Wraparound systems of care and child welfare
- Background and driving record checks will be performed
Compensation
The pay range for this position is $22.84 to $26 per hour. The actual compensation may vary based on experience, education, licensure, certifications, and qualifications.
