Intensive Care Coordinator

About the role

JRI is seeking an Intensive Care Coordinator (ICC) to support youth under the age of 21 with serious emotional disturbances. The ideal candidate will have a talent for connecting with youth and their families and a desire to make a positive impact.

Responsibilities

• Facilitate Care Planning Teams
• Coordinate multiple services/supports for MassHealth-enrolled children and youth
• Act as a liaison with individuals, agencies, and programs
• Identify, assist families in accessing, and monitor available services and natural supports
• Provide trauma-informed, wrap-around services that center the needs of the youth
• Support parents and caregivers in learning effective advocacy skills

Requirements

• Bachelor’s or Master’s degree in social work, psychology, or related human service field
• Daily access to reliable, insured vehicle and active Driver’s License
• Flexible work schedule, including evenings and weekends
• Experience navigating child/family-serving systems and working collaboratively with state agencies, consumer advocacy groups, and behavioral health facilities
• Experience in working with and advocating for children/families with serious emotional/behavioral health needs
• Knowledge of Wraparound systems of care and child welfare
• Background and driving record checks will be performed

Compensation

The pay range for this position is $22.84 to $26 per hour. The actual compensation may vary based on experience, education, licensure, certifications, and qualifications.
