YOUR TOP RESPONSIBILITIES

  • Conduct initial assessments of potential clients, gathering all relevant information including a comprehensive inspection, inventory, and documenting of the client's possessions.
  • Input accurate and complete client data into the Mission’s database.
  • Cook up support services for clients, such as medical, legal, and ancillary services, to remove potential obstacles and enhance the potential for client success.
  • Handle emergencies and crises as they arise, following established protocols.
  • Responsible for conducting case management assessments on community, shelter, and program clients, and assisting for proper program placement.
  • Transporting clients when necessary.
  • Distributing medication as needed.
  • Demonstrate understanding and comply with all relevant legal, regulatory, and compliance requirements, company policies and procedures, and the Code of Ethical Conduct.
  • Follow the Mission’s Statement of Faith and lifestyle and adhere to the Mission’s employee policy manual and guidelines set forth by the CEO.
  • Leading or participating in devotions, prayers or other organization activities that promote a credible Christian witness and lifestyle.
  • Perform other duties within scope as assigned.

Requirements

  • Bachelor’s degree from an accredited university or college in social work, sociology, psychology, counseling, or related social service field or 4 years of experience in case management, or related field, or an equivalent combination of education and experience required.
  • Certification in Peer Support is highly desired.
  • Proficient in Microsoft Office software.
  • Program database experience preferred.
