Intake Coordinator - PROS

About the role

RiseWell Community Services is seeking a compassionate Full-Time Intake Coordinator to join our PROS Program- Personalized Recovery Oriented Services in Patchogue, NY.

Responsibilities

• Screens and conducts intakes for individuals seeking admission to the PROS Program.
• Conducts assessments to determine individual’s needs and preferences and develops recommendation for the person’s Individual Recovery Plan.
• Conducts new member orientation and coordinates with all referral sources and collaterals.
• Maintains appropriate records according to program regulations.

Qualifications

• Licensed Master Social Worker.
• Experience working with individuals in recovery from Mental Illness.
• Valid and clean NYS Drivers License.
• Fingerprinting, criminal record check, approval from NYS Office of Mental Health required.
