About the Role
Guide patients and families through the hospice admission process, ensuring smooth transitions into care.
Responsibilities
- Review referrals and medical records to verify eligibility.
- Coordinate urgent or same-day admissions as needed.
- Educate families on hospice services, answer questions, and address concerns.
- Coordinate services, equipment, and care needs with internal teams and external providers.
- Verify insurance, obtain authorizations, and document all referral and patient information.
- Communicate with healthcare professionals to ensure continuity of care.
- Follow regulatory and compliance requirements throughout the intake process.
- Serve as a resource for hospice criteria and support referral sources with alternatives if necessary.
Requirements
- Licensed Practical Nurse (LPN) or Registered Nurse (RN) with current licensure.
- Minimum of two years healthcare experience, preferably in hospice operations.
- Strong clinical judgment and communication skills.
- Ability to coordinate care and advocate for patients and families.
