Intake Coordinator - Hospice LPN
Responsibilities
- Guide patients and families through the hospice intake and admission process with care and clarity.
- Review referrals and medical records to ensure patients meet hospice eligibility.
- Triage incoming referrals and coordinate urgent or same-day admissions when needed.
- Educate patients and families on hospice services, answering questions and easing concerns.

- Communicate with physicians, discharge planners, and other healthcare professionals to ensure continuity of care.
- Ensure all regulatory and compliance steps are followed throughout the intake process.
- Serve as a knowledgeable resource for hospice criteria and support referral sources with alternatives if needed.
- Registered Nurse or Licensed Practical Nurse, with current and unencumbered state licensure.
- At least two years of healthcare experience, preferably in hospice operations.
