Intake Coordinator - LPN (Home Health)

Role Overview: As an LPN Intake Coordinator, you will handle incoming referral and order calls, gather and verify patient demographic and clinical information, and ensure accurate communication with relevant teams. Key Responsibilities Obtain and compile referral data from clients, physicians, and referral sourcesVerify physician licenses and research coverage or pay sources for referralsEducate patients, families, physicians, and hospitals about home health servicesAct as a liaison between clinical and marketing teams Qualifications Current, unrestricted LPN license in the practice stateAt least 1 year of insurance or medical experienceValid driver’s license, vehicle insurance, and access to transportation Preferred Skills: CPR certification or ability to obtain within 90 days Compensation & Benefits: Hourly pay ranges from $14.00 to $24.90, based on experience and location.
