Intake Coordinator - LPN (Home Health)

About the Role

The LPN Intake Coordinator is responsible for managing incoming referrals and orders, communicating patient information, and coordinating with clinical and marketing teams to ensure smooth processes and effective communication.

Responsibilities

• Obtain and verify demographic and clinical information from clients, physicians, and referral sources.
• Research coverage, verify pay sources, and confirm physician licenses.
• Educate patients, families, physicians, and hospitals about home health services.
• Serve as a liaison between clinical and marketing staff to facilitate smooth communication and process flow.

Qualifications & Skills

• Current, unrestricted LPN licensure in the practice state.
• At least 1 year of insurance or medical experience.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.
• Preferred: CPR certification or ability to obtain within 90 days.
