About the role
Responsible for receiving and analyzing referral and order calls, effectively communicating patient and referral information to relevant teams.
Responsibilities
- Obtain demographic and clinical data from clients, physicians, and referral sources to compile referrals.
- Verify licensing and research coverage or pay source, including commercial policies and HIQA.
- Educate patients, families, physicians, and hospitals about available home health services.
- Act as a liaison between clinical and marketing staff.
Requirements
- Current, unrestricted LPN licensure in the state of practice.
- At least 1 year of insurance/medical experience.
- Valid driver’s license, vehicle insurance, and access to transportation.
- Preferred: CPR certification or ability to obtain within 90 days.
- Must pass a drug test prior to employment.

Pay
Compensation is hourly, ranging from $14.00 to $24.90.
