About the Role

The San Francisco Intake Services Program works with high-risk populations from a variety of backgrounds. Our programs are open and welcoming to adult participants of any gender identity. We accept the majority of participants who seek Outpatient treatment, including walk-ins, referrals from other agencies, and step-down transitional clients from residential treatment programs.

The Intake Coordinator I is an integral part of an interdisciplinary team that supports our clients through the assessment of their needs and strengths in support of their health and recovery. The Intake Coordinator I will work in a collaborative environment to connect clients to the level of treatment they need while managing a small caseload of individual clients or facilitating group treatments. Since many of our clients have backgrounds in the criminal justice system, are chronically homeless, and/or otherwise economically disenfranchised, the Intake Coordinator I will show flexibility in meeting the changing needs of our programs, tolerate working in a fast-paced variable environment, and assess clients for harm to self and others. They will diagnose and treat clients with co-occurring disorders.

Responsibilities

Customer Service and Communication

• Welcomes clients to HealthRIGHT 360 substance abuse, mental health, and gambling services and provides high-quality customer service.
• Conducts face-to-face interviews with clients to screen for appropriate treatment needs.
• Communicates and coordinates with outside referral agencies and stakeholders via phone, fax, email, and in person.

Data Entry and Compliance

• Ensures all required admission paperwork is completed accurately and entered in a timely manner in multiple electronic data systems.
• Makes program placements based on client needs and bed availability.
• Maintains client triage/waitlist and conducts daily overview of program services.

Screening and Assessments

• Identifies and evaluates an individual's strengths, weaknesses, problems, and needs for the development of a treatment plan.
• Conducts level of care assessments to help determine eligibility for withdrawal management and residential substance use disorder treatment services.
• Utilizes the agency’s assessment tools to gather client information for admissions directly from the client.
• Provides mental health/level of care assessments and diagnoses for adults seeking residential services.
• Consults regularly with the Director to ensure clients meet the criteria for safe admission to the appropriate level of residential care.
• Provides crisis intervention and referrals to high-level of care as necessary.
• Performs mental health/level of care assessments with inmates at San Francisco County Jail sites seeking residential treatment upon release.
• Manages a small individual therapy caseload or facilitates group therapy treatment.
• Provides thorough written documentation of all services provided in compliance with regulations, contracts, and behavioral health standards.
• Persists in efforts to solve issues even when faced with internal barriers; takes personal responsibility for customer service outcomes.
• Performs other duties as assigned.

Requirements

• Drug and Alcohol Registration from an accredited credentialing body (CADTP, CCAPP, CAADE).
• Minimum of two years of experience in dual diagnosis substance abuse treatment in an inpatient or outpatient community mental health setting (preferred).
• Experience conducting mental health assessments and providing appropriate diagnoses.
• Experience with mental health Medi-Cal procedures.
• Demonstrated clinical paperwork, timely record-keeping abilities, and dual entry in EHR systems.
• Valid driver’s license and ability to pass an agency driving test to use an agency vehicle for off-site visits.
• Drug and Alcohol Certification from an accredited credentialing body (CADTP, CCAPP, CAADE).

Skills and Knowledge

• Familiarity with Drug-Medical and ASAM Criteria.
• Familiarity with AVATAR.
• Solid understanding of diagnostic criteria for ICD-10/DSM V classification of mental disorders.
