Intake Coordinator I

Essential Functions And Responsibilities

• Counsels and screens clients via phone and face-to-face for veterans, guardsman, active-duty, and their families.
• Supports clients in completing initial consent forms and releases as necessary.
• Coordinates case assignments with the multi-disciplinary team.
• Thoroughly and efficiently documents interventions or communications within 48 hours.
• Collaborates with the clinical team and attends multidisciplinary team meetings.
• Provides crisis management support to clients and the team as needed.
• Performs services both in-person and via telehealth platforms, as required.

Qualifications

• Master’s degree in social work, sociology, psychology, or a related field is required.
• Oklahoma Licensure or candidacy for licensure in one of the following is preferred: Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), Licensed Alcohol and Drug Counselor (LADC/MH).
• At least one year of experience providing intake coordination and/or case management services, with preference for experience working with military or veteran populations.
• Knowledge of military-connected and veteran service organizations and resources.
• Excellent written and oral communication skills.
• Highly organized, detail-oriented, and reliable.
• Works effectively in and contributes to a healthy team environment.
• Proficient in using technology to perform duties.
• Displays appropriate understanding of and adherence to policies and procedures and organizational hierarchy.
• Able to set priorities and evaluate and create solutions to work-related problems.
• Able to stand or sit for long periods of time, lift, push, or pull up to 25 pounds, travel approximately 750% of the time, use various office equipment, and maintain appropriate personal hygiene.
