About the role

The Intake and Eligibility Manager oversees all departmental responsibilities and processes as defined by He-M 505, including receiving referrals, assisting individuals and their families or legal representatives in completing eligibility applications, and processing applications for eligibility determination in accordance with state laws and regulations.

The Manager conducts comprehensive file reviews and assessments to determine eligibility, oversees transfers to and from Region 7, and facilitates standardized functional evaluations when needed. This role is also responsible for data collection and tracking related to referrals, intakes, eligible and ineligible determinations, and other assigned metrics.

Responsibilities

  • Provide office presence and availability during business hours to receive and respond to referrals from potential applicants and external stakeholders, tracking relevant data.
  • Assist applicants, families, and/or client representatives in completing written applications for eligibility determination in accordance with He-M 503.04, He-M 522.03(a), and Area Agency standards and policies.
  • Guide individuals, families, or guardians in obtaining pertinent records (evaluations, assessments) and other relevant documentation to support the application or determine eligibility.
  • Review each application and relevant records to identify necessary data or provide a list of missing information.
  • Ensure adherence to eligibility determination timelines stated in He-M 503.05(d) and He-M 522.06(a), apply for extensions when appropriate, and document actions and timelines accordingly.
  • Arrange for and coordinate comprehensive screening and evaluation of eligible clients in accordance with He-M 503.05 and He-M 522.06(a), which may include facilitating standardized interviews.
  • Meet with individuals, families, or guardians to determine needed services and approximate time frames (e.g., Case Management, Family Support, Day Program).
  • Notify applicants or representatives of eligibility findings in writing (He-M 503.05(h) and He-M 522.06(b)).
  • Remain informed of the appeals process, represent the Agency at appeals, meet with legal counsel, gather additional documentation, prepare and file documentation with the Administrative Appeals Unit, and attend hearings.
  • Demonstrate a working knowledge of Medicaid applications and Probate Court guardianship petitions, including understanding legal roles defined therein.
  • Refer clients to appropriate internal or external community resources.
  • Ensure clients’ needs and rights during the referral and eligibility process are considered and documented.
  • Understand initial service planning steps, such as referring eligible clients with families to the Family Support department to determine the need for respite services or other support.
  • Maintain documentation, data entry, and storage processes as defined by the Agency.
  • Oversee the completion of the transfer process between regions, including reviewing documentation, meeting with identified parties, and gathering additional documentation as needed.
  • Perform other duties and responsibilities as assigned by the Supervisor.

Requirements

  • Master’s Degree preferred; Bachelor’s Degree required, along with experience in the Human Services field.
  • Minimum of 2 years’ experience working with developmentally disabled persons preferred; sensitivity to the needs of the Intellectual Disability population, their families, and representatives.
  • Proficiency in Microsoft Office Suite and database systems.
  • $100,000/300,000 Automobile Liability Insurance.
  • Valid driver’s license and access to a safe operating vehicle during scheduled working hours.

  • Demonstrate creative, innovative thinking and problem-solving, including delegation of relevant administrative tasks.

Pay

Salary range: $50,000 – $60,000 per year (based on experience and qualifications).
