About the role
LPN/LVN providing bedside care to adult rehabilitation patients.
Responsibilities
- Administer medications, perform treatments, and monitor vital signs.
- Support activities of daily living (ADLs) under RN and physician guidance.
- Accurately document patient information and progress.
- Collaborate with an interdisciplinary rehabilitation team.
- Educate patients and families on care plans and safety protocols.
- Contribute to developing and implementing individualized care plans.
- Provide compassionate, patient-centered care to those recovering from stroke, brain and spinal cord injuries, orthopedic, and neurological conditions.
Qualifications & Skills
- Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) licensure.
- Strong clinical, communication, and teamwork skills.
- Ability to support patient recovery and safety in a rehab setting.
