Responsibilities
- Provide evidence-based, patient-centered care for adult patients recovering from illness, injury, or surgery.
- Assess and monitor patient conditions, manage medications and complex care needs.
- Support mobility and functional recovery, and educate patients and families on recovery plans.
- Collaborate with physicians, therapists, and case managers to develop individualized care plans.
- Promote safety, prevent complications, and document progress using advanced EHR systems.
Qualifications & Skills
- Strong assessment and monitoring skills.
- Ability to manage complex care needs and medications.
- Excellent communication and collaboration skills for interdisciplinary teamwork.
- Proficiency with electronic health record systems.
