Inpatient Medical Social Worker

Responsibilities

• Provide comprehensive social work services, including assessments, care planning, interventions, and discharge coordination to meet patients’ psychosocial needs.
• Collaborate with multidisciplinary teams to ensure safe, timely care transitions and access to services such as post-acute care, mental health support, and community resources.
• Educate and advocate for patients and families, connecting them with appropriate services, and addressing issues like end-of-life planning, abuse, and social needs.
• Manage care progression to optimize outcomes, reduce readmissions, and ensure appropriate levels of care.
• Maintain documentation, compliance with regulations, and professional standards, while supporting staff training and continuing education.

Requirements

• Master’s of Social Work from an accredited school.
• 1-2 years of experience in a clinical, hospital setting post licensure preferred.
• Licensure/Registration/Certification:
  • For IL locations: LSW Eligible, LCSW preferred issued by the State of Illinois.
  • For WI locations: Social Work Certification issued by the State of Wisconsin.

Schedule

This position is part of a job share, 40 hours over 2 weeks, with the following details:

• Shift: 7:30 AM - 4:00 PM.
• Weekend rotation: approximately 6 per year.
• On-Call rotation: approximately 15 days per year.
• Holiday rotation: no more than 1 holiday per year.

Physical Requirements and Working Conditions

• This position requires travel, exposing you to weather and road conditions.
• Operates all equipment necessary to perform the job.
• Exposed to a normal office environment and patient care environment.
• May be exposed to hazardous materials and life-threatening diseases; must abide by personal protective equipment as ordered.
• Requires sitting, standing, speaking, reading/writing, and walking throughout the workday.
• Fast-paced work environment with established time constraints.
