Inpatient Care Manager - MSW

About the Role

This position involves managing comprehensive care plans for inpatient populations, ensuring safe and timely transitions of care while advocating for patient and family needs.

Responsibilities

• Conduct comprehensive assessments upon patient admission to develop individualized care and discharge plans.
• Coordinate safe and timely discharge processes, including securing authorizations, arranging transportation, and facilitating transfers.
• Monitor patient progress through daily reviews and participate in multidisciplinary rounds to address barriers to discharge.
• Advocate for patients and families by addressing clinical, educational, and psychosocial needs, ensuring their preferences are incorporated into care plans.
• Collaborate with healthcare teams and educate patients/families on self-management and adherence to care plans.
• Maintain knowledge of payer benefits, reimbursement, and healthcare regulations.
• Communicate effectively with physicians, patients, families, and staff to coordinate transitions of care.
• Support performance improvement initiatives and resource management to enhance patient outcomes and system efficiency.

Requirements

• Master’s Degree in Social Work.
• Less than 1 year of experience in an MSW internship program.
• Physical ability to perform activities including lifting, kneeling, bending, and CPR if required.

Skills

• Ability to interact with diverse populations and set priorities efficiently.
• Knowledge of computer applications and analytical tools.
• Proven leadership skills.
• Preferred: Familiarity with EHR systems like EPIC; certifications such as ACM or CCM within 3 years.
