About the Role
The Inpatient Care Manager-MSWork coordinates comprehensive patient care across the continuum, including assessment, planning, implementation, and evaluation of individualized care plans.
Responsibilities
- Conduct thorough assessments upon patient admission to determine needs, length of stay, and discharge planning requirements.
- Develop, implement, and monitor care and transition plans in collaboration with multidisciplinary teams.
- Coordinate safe and timely discharges by securing authorizations, arranging transportation, homecare, and facility placements.
- Participate in daily reviews and multidisciplinary rounds to manage patient progress and address discharge barriers.
- Advocate for patients and families, addressing clinical, educational, and psychosocial needs, ensuring their preferences are integrated into care.
- Collaborate with healthcare team members and educate patients/families on self-management and adherence to care plans.
- Maintain knowledge of payer benefits, reimbursement processes, and support performance improvement initiatives.
Requirements
- Master’s Degree in Social Work (MSW).
- Less than 1 year of experience in an MSW internship program.
- Strong ability to interact with diverse populations and set priorities for care coordination.
- Proficiency in computer applications and analytical tools.
- Proven leadership capabilities.
Preferred Qualifications
- Familiarity with EHR systems like EPIC.
- Certifications such as ACM or CCM within 3 years.
