About the Role

The Inpatient Care Manager-MSWork coordinates comprehensive patient care across the continuum, including assessment, planning, implementation, and evaluation of individualized care plans.

Responsibilities

  • Conduct thorough assessments upon patient admission to determine needs, length of stay, and discharge planning requirements.
  • Develop, implement, and monitor care and transition plans in collaboration with multidisciplinary teams.
  • Coordinate safe and timely discharges by securing authorizations, arranging transportation, homecare, and facility placements.
  • Participate in daily reviews and multidisciplinary rounds to manage patient progress and address discharge barriers.
  • Advocate for patients and families, addressing clinical, educational, and psychosocial needs, ensuring their preferences are integrated into care.
  • Collaborate with healthcare team members and educate patients/families on self-management and adherence to care plans.
  • Maintain knowledge of payer benefits, reimbursement processes, and support performance improvement initiatives.

Requirements

  • Master’s Degree in Social Work (MSW).
  • Less than 1 year of experience in an MSW internship program.
  • Strong ability to interact with diverse populations and set priorities for care coordination.
  • Proficiency in computer applications and analytical tools.
  • Proven leadership capabilities.

Preferred Qualifications

  • Familiarity with EHR systems like EPIC.
  • Certifications such as ACM or CCM within 3 years.
