About the team

The Integrated Case Management team at Sentara Martha Jefferson has 16 members including MSW and RN Case Management staff. This is a collaborative environment with a patient-first mindset. The average caseload is 15 patients per day, and a strong sense of urgency is preferred. Hours are primarily between 8:00 AM and 4:30 PM with some variation and one weekend shift every 6 to 8 weeks.

Responsibilities

• Provide medical social work services to patients in the hospital setting.
• Assess the social, cultural, environmental, and financial situations, as well as the disposition needs of each referred patient.
• Collaborate with healthcare team members, patients, and community organizations to identify needs and resources in the hospital and community.
• Develop effective transition plans in consultation with the healthcare team.
• Collaborate with the RN Case Manager to ensure a smooth flow of patients through their hospital stay.
• Function in hospital-based practice settings.

Requirements

• Master’s degree in Social Work.
• Certification in Case Management (CCM, CMAC, or ACM-SW) required within one year of eligibility.
• BLS certification required within 90 days of hire.
• 1 year of acute care and/or healthcare experience, including educational internship experience in an acute care facility.
• Experience with community resources is preferred.
• Ability to develop effective transition plans and collaborate in a team environment.
