Inpatient Care Manager - MSW

About the Role

This position involves managing individualized care plans for a designated inpatient population, ensuring safe and timely discharge planning, and advocating for patients and families.

Responsibilities

• Assess, plan, implement, and evaluate individualized care plans for patients.
• Coordinate safe and timely discharge planning, including securing authorizations, arranging transportation, and facilitating transfers.
• Monitor patient progress, participate in multidisciplinary rounds, and resolve barriers to discharge.
• Advocate for patients and families by addressing clinical, educational, and psychosocial needs.
• Collaborate with healthcare teams and support patient and family involvement in care planning.
• Develop knowledge of payer benefits, assist with self-management education, and promote adherence to care plans.
• Communicate effectively with physicians, patients, families, and team members to ensure coordinated transitions.

Requirements

• Master’s Degree in Social Work with less than 1 year of internship experience.
• Ability to interact with diverse populations and set priorities efficiently.
• Knowledge of computer applications, analytical tools, and healthcare reimbursement systems.
• Proven leadership qualities.

Preferred Qualifications

• Familiarity with EHRs (EPIC).
• Certifications such as ACM or CCM within 3 years.

Physical Demands

• Frequent sitting, standing, and keyboard use.
• Activities may include kneeling or bending.
• Ability to lift up to 25 lbs.
