About the role
Work one-on-one with individuals with Intellectual and Developmental Disabilities (I/DD) to help them achieve their goals and become as independent as possible. This can be done in either a private home or within the community.
Responsibilities
- Implement individual habitation plans to teach daily living skills, socialization, employment, leisure, and recreational activities.
- Document progress in progress notes, which should include observations, consumer participation, and any relevant information.
- Obtain guardian’s signatures on documentation as required.
- Supervise and assist with general housekeeping, meal preparation, grooming, bathing, eating, and other activities as necessary.
- Provide transportation to community services/activities as needed.
- Aid with money management and encourage decision-making.
- Assist with prescribed exercises and maintain therapeutic/adaptive equipment.
- Report any changes in the person’s condition, needs, or circumstances.
- Report any allegations of abuse, neglect, misappropriation, fraud, or drug diversion.
- Perform other duties as required by circumstances.
Requirements
- At least 21 years old
- High School Diploma or GED
- Ability to read, write, understand, and follow directions
- No substantiated findings on the NC Health Care Abuse Registry
- LME/MCO contract compliance with SBI and DMV requirements
- Valid driver’s license and 3 full years of driving experience
- Acceptable level of automobile liability insurance
- Experience with persons with I/DD preferred but not required
- Willingness to complete all required trainings as assigned
- Access to a device with internet connection at the time services are rendered
