About the role

We are seeking an Information & Referral Specialist to provide information to our consumers in response to service and eligibility inquiries.

Responsibilities

  • Assess the health and functional status of members
  • Develop community-based care plans and related service packages
  • Participate in the development of a member's Individual Care Plan (ICP)
  • Arrange, coordinate, and monitor the provision of appropriate members
  • participate in the determination of a member's need for institutional and community long term care services

Requirements

  • Your passion for helping others and desire to make a difference.
  • Bachelor's degree in social work or related field highly preferred; current licensure if appropriate
  • Minimum of two years' professional experience in the care of people 65 and older
  • Current Social Work license (LSW) or higher preferred
  • Working knowledge of social, emotional, and physical process of aging
  • Ability to obtain information of a personal nature through interviews
  • Ability to gain and hold the confidence of clients and their families
  • Draw appropriate conclusions and make recommendations
  • Knowledge and skill necessary to work successfully within an interdisciplinary team
  • Excellent written and verbal skills
  • Excellent computer skills
  • Proficient with Microsoft Office Suite or related software

Qualifications

Our ideal candidate will have:

  • A passion for helping others and making a difference
  • A Bachelor's degree in social work or a related field (preferred)
  • At least two years of professional experience caring for people 65 and older
  • A current Social Work license (LSW or higher preferred)
  • A strong understanding of the social, emotional, and physical processes of aging
  • The ability to gather sensitive personal information through interviews
  • The capability to build trust and maintain confidence with clients and their families
  • The ability to analyze situations and provide recommendations
  • Adeptness in working within an interdisciplinary team
  • Outstanding communication skills both written and verbal
  • Expertise with Microsoft Office Suite or equivalent software
