About the role
We are seeking an Information & Referral Specialist to provide information to our consumers in response to service and eligibility inquiries.
Responsibilities
- Assess the health and functional status of members
- Develop community-based care plans and related service packages
- Participate in the development of a member's Individual Care Plan (ICP)
- Arrange, coordinate, and monitor the provision of appropriate members
- participate in the determination of a member's need for institutional and community long term care services
Requirements
- Your passion for helping others and desire to make a difference.
- Bachelor's degree in social work or related field highly preferred; current licensure if appropriate
- Minimum of two years' professional experience in the care of people 65 and older
- Current Social Work license (LSW) or higher preferred
- Working knowledge of social, emotional, and physical process of aging
- Ability to obtain information of a personal nature through interviews
- Ability to gain and hold the confidence of clients and their families
- Draw appropriate conclusions and make recommendations
- Knowledge and skill necessary to work successfully within an interdisciplinary team
- Excellent written and verbal skills
- Excellent computer skills
- Proficient with Microsoft Office Suite or related software
Qualifications
Our ideal candidate will have:
- A passion for helping others and making a difference
- A Bachelor's degree in social work or a related field (preferred)
- At least two years of professional experience caring for people 65 and older
- A current Social Work license (LSW or higher preferred)
- A strong understanding of the social, emotional, and physical processes of aging
- The ability to gather sensitive personal information through interviews
- The capability to build trust and maintain confidence with clients and their families
- The ability to analyze situations and provide recommendations
- Adeptness in working within an interdisciplinary team
- Outstanding communication skills both written and verbal
- Expertise with Microsoft Office Suite or equivalent software
