Information and Referral Specialist
About the role
OCES is growing! We are expanding to 11 more towns covering the south shore area, including Quincy, Braintree, Weymouth, etc. OCES supports older adults and individuals with disabilities by providing vital information and coordination of services. By promoting healthy, safe living for our consumers, we hope to allow them to stay active and engaged in their lives and in their own communities for as long as possible. Responsibilities
- Assess the health and functional status of members
- Develop community-based care plans and related service packages
- Participate in the development of a member's Individual Care Plan (ICP)
- Arrange, coordinate, and monitor the provision of appropriate services
- Participate in the determination of a member's need for institutional and community long-term care services
Requirements
- Your passion for helping others and desire to make a difference
- Bachelor's degree in social work or related field highly preferred; current licensure if appropriate
- Minimum of two years' professional experience in the care of people 65 and older
- Current Social Work license (LSW) or higher preferred
- Working knowledge of social, emotional, and physical processes of aging
- Ability to obtain information of a personal nature through interviews
- Ability to gain and hold the confidence of clients and their families
- Draw appropriate conclusions and make recommendations
- Knowledge and skill necessary to work successfully within an interdisciplinary team
- Excellent written and verbal skills
- Excellent computer skills
- Proficient with Microsoft Office Suite or related software
