Independent Living Program Manager

Full-time, benefit-eligible position located in Wellesley and Newton, MA. Supervised by the P2P Program Director.

About the Role

Pathway to Possible is a congregate care facility providing independent living and assisted living for adults with developmental disabilities. The program offers housing and support services to adults referred by the Department of Developmental Services. Program goals include:

• Developing skills and abilities needed to be productive members of their residential program
• Developing skills for independent living where possible
• Establishing and promoting lifelong connections

Responsibilities

• Follow all policies and procedures as outlined in the Pathway to Possible (P2P) Personnel Policies and Procedures and the P2P Policies to Promote Human Dignity
• Provide regular documented supervision and feedback to case management staff to ensure the safety and well-being of all clients
• Schedule and approve staffing to meet individual contract requirements
• Provide up to 10 weeks of on-call coverage per year
• Actively participate in agency-wide events, staff meetings, and training as a member of the management team
• Ensure all client health care and medical needs are met
• Maintain thorough knowledge of individuals’ financial resources, including government benefits, housing subsidies, and other entitlements
• Ensure client compliance with entitlement benefits
• Communicate and implement all safety programs and policies consistently, including maintaining accurate and current safety plans
• Ensure ISP plans are developed and implemented in accordance with P2P and DDS guidelines
• Ensure proper monthly documentation is completed
• Direct case managers to oversee the monitoring and scheduling of medical/dental appointments for each resident
• Participate in P2P and Department of Developmental Services training programs and attend all Program Manager meetings
• Monitor and coordinate with HMO to ensure proper operation of house, grounds, vehicles, furnishings, equipment, and physical plant
• Maintain all program records according to established guidelines and state and federal requirements
• Maintain financial accounts and budgetary guidelines in alignment with state, federal, and P2P guidelines
• Accurately document and report monthly billing based on hours of service received by each individual
• Provide an appropriate number of service hours for each individual based on their DDS contracts and individual needs
• Maintain relationships with involved families, guardians, and other providers
• Train ILP case managers in HCSIS, ICENTRIX, and client banking

Requirements

• Bachelor's degree in human services or related field
• A graduate degree and/or 5 years of program management experience is preferred
• Commitment to the empowerment of individuals with intellectual and developmental disabilities
• Familiarity with the DDS delivery system
• Valid Massachusetts Driver's License
• Current MAP certification preferred
• First Aid and CPR certification required
• Experience with individuals who have complicated medical needs is preferred
• Proficient with Microsoft Office Suite or related software
• Legally authorized to work in the United States
• Clearance of CORI and SORI background checks as required by state and federal law
• Clearance of DPPC registry as required by state law
• Cleared search of fingerprinting through the DDS criminal background check

Physical Requirements

• Must be able to lift up to 15 pounds
• Must be able to provide physical assistance to adults using wheelchairs
• Must be able to evacuate individuals supported within 2 ½ minutes
• Must be able to assist individuals in transferring from sitting to standing

Other Requirements

• Must maintain MAP certification for the duration of employment
• Comfortable working in an environment with a therapeutic cat

Pay

$65,000 - $70,000 annually

40 hours per week, full-time (Monday – Friday, 10:00 AM – 6:00 PM)
