Independent Living Case Manager

About the role

Damar Services is seeking an Independent Living Case Manager to support young people in developing essential life skills. The role involves providing educational, vocational, and employment assistance, along with health, housing, and social skills development.

Responsibilities

• Provide age-appropriate education on life skills, including tutoring, mentoring, health care, and personal relationship building.
• Teach clients how to maintain a safe and healthy living environment, including tasks like cleaning, laundry, and housekeeping.
• Document client progress accurately and consistently, including medication logs, incident reports, and progress notes.
• Attend mandatory staff meetings and other meetings as assigned.
• Operate a personal vehicle to transport clients or perform other work-related duties.
• Participate in data collection and use data to improve outcomes, practices, and service delivery.
• Implement and update independent living plans to promote client independence.
• Monitor clients' conditions and behaviors to assess goal achievement.
• Adhere to policies and procedures, including those related to client service documentation.
• Maintain financial responsibility and ensure efficient use of resources.
• Collaborate with clients, families, professionals, and the community to provide optimal experimental learning experiences.
• Show respect for cultural and ethnic diversity in all interactions.

Requirements

• Bachelor’s degree in social work, psychology, sociology, or a directly related human service field.
• Minimum of 3 years of relevant experience in this field.
• Minimum age of 21 to meet licensure requirements.
• Valid Indiana driver’s license and a clean driving record.
• CPR certification.
• Clear background check and no felony convictions or substantiated charges of neglect.

Qualifications

• Ability to pass and maintain CPR certification.
• Valid Indiana driver’s license and driving record meeting Damar’s insurance carrier’s eligibility requirements.
