Independent Living Case Manager
Job Summary
The Independent Living Case Manager is responsible for assisting young people develop the necessary skills via experimental learning in order to enhance self-sufficiency. Services that could be provided to clients include Educational, Vocational and Employment, Health, Housing, Life and Social Skills, and Youth Development. This role also involves providing age-appropriate education regarding life skills needed to live independently, such as tutoring, mentoring, health care, self-esteem building, life interest explorations, and personal relationship development. The Case Manager assists and teaches clients about maintaining a safe and healthy environment, including performing tasks like cleaning, washing clothes, changing linens, folding and sorting laundry. They complete all required documentation professionally, accurately, and thoroughly to ensure consistent and accurate client treatment. The Case Manager attends mandatory staff meetings and other meetings as assigned, operates a personal vehicle to transport clients or perform other work-related duties, actively participates in and oversees data collection, and uses data to improve outcomes, practices, and service delivery.
Education
- Bachelor’s degree in social work, psychology, sociology, or a directly related human service field.
Experience
- 3 years of relevant experience in this field.
Equipment Familiarity
- Proficiency with MS Office applications including: Word, Excel, Outlook, and PowerPoint.
Certifications, Licenses Or Special Training
- Minimum age of 21 per licensure requirements.
- Ability to pass and maintain CPR certification.
Background Screening
- Must meet and maintain background screening requirements for all Damar Programs as detailed in policy 1.HR.02 Pre-Employment and Employee Background Checks.
Driving Requirements
- Valid Indiana driver’s license and driving record that meets eligibility requirements of Damar’s insurance carrier (for all positions requiring driving).
Physical Requirements
- Successful completion of pre-employment physical required.
