In-Home Traveler Nurse Practitioner or Physician Assistant

Responsibilities

• Conduct comprehensive health evaluations for Medicare and vulnerable populations in patient homes.
• Perform head-to-toe assessments, review medical histories, and reconcile medications.
• Identify care gaps using advanced technology platforms.
• Collaborate with interdisciplinary teams to ensure coordinated care.
• Document findings accurately in electronic health records (EHR).
• Support care coordination and provide patient education to enhance value-based care.

Qualifications & Skills

• Licensed Nurse Practitioner or Physician Assistant with relevant clinical experience.
• Strong assessment, clinical, and documentation skills.
• Ability to work independently with autonomy in the field.
• Effective communication and teamwork abilities.
• Comfort with using technology for documentation and care management.
