In-Home Traveler Nurse Practitioner or Physician Assistant (Full Time)

About the role

This full‑time clinician position provides in‑home health evaluations as a Nurse Practitioner or Physician Assistant. The clinician travels within designated areas—sometimes requiring overnight stays—to conduct 45‑minute visits that include medical history reviews, physical assessments, and diagnostic tests when needed. The goal is to improve health outcomes and bridge care gaps for patients in their own homes.

Responsibilities

• Build trusted relationships with patients in their homes to support personalized health assessments.
• Conduct virtual visits as necessary.
• Utilize an iPad and clinical workflow to document evaluations.
• Provide diagnostic and preventive services, including screenings.
• Travel locally and within licensed states based on business needs, including potential overnight stays.

Qualifications

• Active, unrestricted licenses in the required states.
• Board certification as a Nurse Practitioner (NP) or Physician Assistant (PA).
• Ability to obtain and maintain multi‑state licenses.
