About the Role
As the In-Home Services RN Manager, you will ensure the completion of initial and annual assessments of participants in their home environment to determine their individual needs and coordinate plans of care (POC). You will also supervise home care staff and their functions, with physical attendance at an InnovAge facility or participant home essential to performing job duties.
Responsibilities
- Supervision & Administrative – 90%
- Supervise and manage direct reports, providing leadership and motivation.
- Carry out supervisory responsibilities in accordance with organizational policies and applicable laws, including interviewing, hiring, training, planning, assigning work, appraising performance, and addressing complaints.
- Ensure annual employee performance reviews are conducted timely and effectively.
- Evaluate employee performance for compliance with policies and objectives.
- Foster a positive department tone to maintain morale, teamwork, and professionalism.
- Schedule regular leadership, department, and staff 1:1 meetings for coordination and collaboration.
- Oversee team expenditures, payroll, and overtime, ensuring appropriate scheduling to limit overtime.
- Ensure quality care delivery through participant/caregiver inquiries, care plan reviews, and data trending.
- Serve as a member of the Multi-disciplinary Team and attend meetings as appropriate.
- Participant Care – 10%
- Perform initial assessments of participants in their home to determine needs and coordinate POC with the scheduling department.
- Coordinate personal care activities for participants receiving home care services.
- Provide skilled supportive and restorative nursing care as needed.
- Re-evaluate participants' care plans and health status on an ongoing basis.
- Provide health counseling to participants and families.
- Administer medications, train in self-administration of non-scheduled medications, and monitor compliance.
- Perform annual re-assessments for all IGCP participants to determine appropriateness of in-home services.
- Conduct re-assessments in the home for skilled nursing needs, personal care, and homemaker/chore services.
- Maintain participants' medical records and meet agency charting/reporting requirements.
Requirements
- Current Registered Nurse license required by law.
- 2+ years of RN healthcare experience with an emphasis in geriatrics.
- Experience in direct people leadership, supervising staff, and scheduling.
- Current First Aid and BLS certifications (American Heart Association or American Red Cross).
- Requires transportation, a state-issued driver’s license, good driving record, and auto insurance as required by law.
- Bachelor of Science in Nursing (BSN) required.
- For California: BSN or higher with 3 years of experience in the last 5 years in home health, primary care, or a health facility, including at least 1 year of supervisory or administrative experience or an RN with 4 years of experience in the last 5 years in home health, primary care, or a health facility, including at least 1 year of supervisory or administrative experience.

Pay
Posted pay range: $110,600–$143,800.
