In-Home Nurse Practitioner or Physician Assistant Full Time - Saint Louis, IL

This role supports clinical priorities focused on quality, patient-centered outcomes, and effective care delivery. You will collaborate with clinical leaders, operational teams, and cross-functional stakeholders to improve standards, workflows, and execution. The position offers the opportunity to strengthen care quality, team performance, and measurable outcomes within a healthcare environment.

Responsibilities

• Conduct in-home health evaluations for patients
• Engage in one-on-one interactions to answer health questions
• Perform medication and medical history reviews
• Execute physical evaluations and diagnostics as necessary
• Travel locally and within licensed states for home visits
• Conduct virtual visits when required
• Provide a range of diagnostic and preventive services as indicated

Requirements

• Active, unrestricted license in coverage area(s)
• Board certification required
• Multi-state licenses preferred
• Experience in providing in-home health evaluations
• Strong interpersonal communication skills
• Ability to adapt to diverse home environments
• Familiarity with using digital tools for clinical assessments

Pay

$95,738 – $206,206 annually
