In-Home Nurse Practitioner or Physician Assistant Full Time - Chicago

Location: Remote - US based candidates only, no visa sponsorship available.

Responsibilities

• Conduct in-home health evaluations and assessments
• Build trusted relationships with individuals to enhance health outcomes
• Travel locally and within licensed states as necessary
• Perform virtual visits when required
• Deliver diagnostic and preventive services during evaluations
• Educate patients on managing their health concerns
• Collaborate with a network of clinicians to provide comprehensive care

Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Board certification (required)
• Multi-state licenses (preferred)
• Experience in performing health evaluations
• Strong interpersonal skills for patient engagement
• Ability to work independently in diverse home environments

Pay

$95,738 – $206,206 annually
