In Home Healthcare RN-Low Acuity (Day Shift)

About the role

We are seeking a Registered Nurse (RN) to provide skilled nursing care in home settings, focusing on pediatric patients with feeding tubes and medication needs. The role involves delivering and documenting patient care according to physician plans, assessing patient conditions, and updating care plans as necessary. The RN will educate families, ensure infection control, and adhere to all regulatory standards, including OSHA and state-specific requirements.

Responsibilities

• Providing and coordinating quality patient care based on physician orders
• Monitoring and reporting changes in patient condition
• Documenting assessments, interventions, and responses accurately and timely
• Developing and updating nursing care plans
• Educating patients and families on disease management and self-care
• Maintaining compliance with policies, regulations, and ethical standards

Qualifications and skills

• Active, unencumbered RN license in the practicing state
• Current CPR certification
• Proficiency in clinical assessments, documentation, and compliance
• Six months of nursing experience preferred; home health experience is a plus
• Strong attention to detail, organization, communication, and problem-solving skills
• Physical ability to lift, move patients, and perform clinical duties
