Responsibilities
- Deliver care to patients in their homes, ensuring their needs are met.
- Document patient care electronically using tablets provided in each patient's home.
- Collaborate with other healthcare professionals to ensure comprehensive patient care plans.
- Adhere to all state and federal regulations regarding nursing practice.
Requirements
- Maintain an active, unencumbered license (LPN/LVN) in the state of practice.
- Have current CPR certification (with hands-on component).
- Pass TB skin test (within last 12 months).
- Have six months of hands-on nursing experience (preferred but not required).
- Have reliable transportation.
Qualifications
- Current licensure (LPN/LVN) in the state of practice.
- Compact licenses must be transferred to your state of residence within 90 days.
- Pass TB skin test (within last 12 months).
- Must have reliable transportation.

Monday-Friday (7a-4pm) & Saturday (9am-7pm)
