About the Role

For our client, we are seeking a In-Home Health - Nurse Practitioner Or Physician Assistant (full Time) - Bridgeport CT to join the team of a leader in the Healthcare space. This role will support clinical priorities focused on quality, patient-centered outcomes, and effective care delivery. You will collaborate with clinical leaders, operational teams, and cross-functional stakeholders to improve standards, workflows, and execution. The position offers the opportunity to strengthen care quality, team performance, and measurable outcomes within a healthcare environment.

Location

Remote - US based candidates only, no visa sponsorship available

Compensation

$95,738 – $206,206 annually

Responsibilities

• Conduct in-home health evaluations and engage with patients
• Perform medication and medical history reviews during visits
• Utilize an iPad for clinical documentation and workflow
• Offer additional diagnostic tests as ordered by health plans
• Travel locally and within licensed states as necessary
• Adapt to diverse home environments and assess patient needs
• Provide preventive and diagnostic services as indicated

Qualifications

• Active, unrestricted nurse practitioner or physician assistant license in coverage area(s)
• Board certification is required
• Multi-state licenses are preferred
• Experience in providing in-home health evaluations or similar settings
• Strong interpersonal skills to build trusting relationships with patients
