This full-time role focuses on delivering high-quality in-home health evaluations and preventive care, working collaboratively with a health network to drive meaningful business transformation and improve patient outcomes.

Location: Remote (US-based candidates only, no visa sponsorship available).

Responsibilities

• Conduct in-home health evaluations to review medication and medical history.
• Build rapport with patients to foster trust and improve health outcomes.
• Provide diagnostic and preventive services such as spirometry and diabetic eye exams.
• Flexibly travel locally to patients' homes within licensed states.
• Adapt to conducting virtual visits when necessary.
• Collaborate with fellow clinicians in the health network.
• Maintain professionalism while handling diverse household environments.

Requirements

• Active, unrestricted Nurse Practitioner or Physician Assistant license in coverage areas.
• Board certification required.
• Multi-state licenses preferred.
• Experience in conducting health evaluations.
• Strong interpersonal skills for patient engagement.
• Ability to navigate diverse home environments.
• Comfortable with technology for clinical workflows.

Pay

$95,738 – $206,206 annually.
